GP REFERRAL
All referrals are reviewed and triaged by Dr Zachariah. According to the urgency of your patient’s condition, the referrals are categorised into three levels of priority. Once triaged, our staff will notify the patient and schedule an appointment.
You may send the referral through HealthLink or email, both of which are monitored regularly.
For some patients (Eg: when suspecting deep infiltrating endometriosis, Complex ovarian mass etc), Dr Zachariah might request additional investigations such as a Specialist level pelvic USS before their appointment to facilitate prompt assessment and management.

Referral Requirements
To ensure timely appointments, all referrals must include:
- Date of referral
- Referring GP’s name, address, provider number
- Patient’s full name, address, date of birth and contact number
- Reason for referral
- Investigations including CST, Pelvic USS report, relevant blood test results etc.
Please note: We are a private billing practice and unfortunately can only offer appointments to patients who have private health insurance with active Hospital cover.
